Session Abstract (150 words): 

Determining the post-discharge outcomes of human service recipients presents a complex set of tasks for internal evaluators. Examples include garnering organizational buy-in, interview script and safety protocol development, gathering and tracking participant consent, and methods to increase the interview completion rate.  This demonstration presents one evaluation team’s set of strategies for addressing these steps when implementing and operating a post-discharge outcomes measurement project at a large multiservice agency. The Home for Little Wanderers (The Home) conducts structured follow-up phone interviews up to nine months after services end with legal guardians of children under 18, transition age youth, and adult clients of outpatient, in‑home, school‑based, and out‑of‑home care programs. The presenters will review strategies that have significantly increased The Home’s interview response rate such as standardizing contact protocols; using a variety of methods of contact; and increasing consent form adoption at program sites.
